This eCRF in XForms format demonstrates how the dynamic features of XForms be used to allow the investigator whether to choose to add an Audit Record or not.
In case the eCRF is downloaded from the server as an already existing (already filled) one, the server would blank out the checkbox for Audit Record upon generation of the eCRF, and make the User ID, Location ID and Reason for Change fields mandatory. So in that case, the investigator can never submit the form again without having filled the information required for the Audit Trail.
